Job summary
Angels Care Home Health is seeking a Provider Support Registered Nurse (RN) Case Manager in Tahlequah, OK. The role coordinates comprehensive home care, conducts admission visits, and determines eligibility for services while collaborating with multidisciplinary teams.
Travel across branches is required as part of the coverage plan. The ideal candidate is a licensed RN with CPR, at least one year of RN experience (two preferred), and valid driver’s license with reliable transportation.
Qualifications
- Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
- Current state RN license and CPR certification.
- Willingness to travel to branches within the provider and occasional overnight stays.
- Valid driver’s license and reliable transportation with insurance.
Responsibilities
- Coordinate all aspects of the patient’s home care and interact with other medical disciplines as needed.
- Perform admission visits to patients in their homes and determine eligibility for home care services.
- Develop and follow the plan of care for each patient and update as required.
- Travel to various branches within the provider and support coverage as needed.
