Job summary

Angels Care Home Health is seeking a Provider Support Registered Nurse (RN) Case Manager in McAlester, OK. You will coordinate all aspects of the patient's home care, perform admission visits, and determine eligibility while developing the plan of care.

This full-time role requires RN licensure, CPR, and travel to branches as needed; we value ongoing professional development and a comprehensive benefits package.

Qualifications

• Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse (RN).
• One-year RN experience preferred; two years preferred.
• CPR certification and Hepatitis consent/declination required.
• Valid driver’s license and willingness to travel to branches.

Responsibilities

• Coordinate all aspects of the patient’s home care as the Provider Support RN Case Manager.
• Complete admission visits to homes and determine eligibility for home care services.
• Develop and implement the plan of care with other medical disciplines as needed.
• Travel to provider branches and participate in care coordination.
