Angels Care Home Health in Smithville, OK is seeking a dedicated Registered Nurse Case Manager to coordinate all aspects of patient home care. You will assess patients, collaborate with medical teams, and plan care in the home.

Requirements include RN license, nursing degree, driver’s license, CPR, Hepatitis consent, and at least 1 year of RN experience; reliable transportation and auto liability insurance. We offer competitive salary, PTO, benefits, 401(k), and advancement opportunities.

Qualifications

• Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• One year’s experience as a Registered Nurse, two preferred.
• Proof of CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsabilités

• The RN Case Manager coordinates all aspects of the patient’s home care.
• Interact with other medical disciplines as needed and provide professional care to the patient.
• Perform admission visit to the patient in the home, determine eligibility for home care services, and develop the plan of care to be followed.
