Angels Care Home Health in Lawton, OK is seeking a Registered Nurse (RN) Case Manager to coordinate all aspects of the patient’s home care. You will perform admission visits to establish eligibility and develop the plan of care in collaboration with the patient and family.

The role requires an active RN license, a valid driver’s license, CPR, and Hepatitis consent/declination; one year of RN experience preferred. We offer benefits, vehicle allowance, PTO, and opportunities for growth.

Competenze

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state RN license.
• Current state Driver’s License.
• CPR certification.
• Hepatitis consent/declination.
• One-year experience as a Registered Nurse, two preferred.

• Coordinate all aspects of the patient's home care and perform admission visits.
• Determine the patient's eligibility for home care services and develop the plan of care.
