Job summary

Angels Care Home Health is seeking a Provider Support Registered Nurse (RN) Case Manager to coordinate home care in the Ponca City, OK area. You will liaise with medical teams, perform admission visits, and craft the care plan to meet patient needs.

The position requires an RN license, 1 year of RN experience (2 preferred), CPR, and a willingness to travel across branches with occasional overnights. This full-time role offers benefits and growth opportunities.

Qualifications

• Graduate of an accredited diploma, Associate, or Baccalaureate School of Nursing.
• Current RN license in the state.
• One-year RN experience; two preferred.
• CPR certification and Hepatitis consent/declination.
• Willing to travel to all branches within a provider.
• Occasional overnight stays may be required.
• Valid driver's license and reliable transportation.

Responsibilities

• Coordinate all aspects of home care with medical disciplines.
• Perform admission visits and determine eligibility for home care.
• Develop and follow the plan of care within the designated area.
