HOME CARE REGISTERED NURSE (RN)

Job Description:

1. Participate actively in the interdisciplinary care planning process.
2. Provide professional nursing services according to the plan of care established by the Patient Care Coordinator.
3. Promote favorable outcomes through collaborative practice and the appropriate, cost-effective use of resources.
4. Accurately and thoroughly document services to demonstrate appropriate care delivery, facilitate coordination, justify reimbursement, reduce legal risks, ensure compliance, and support quality improvement.
5. Maintain positive and effective interpersonal relationships.
6. Take personal accountability for completing work assignments and professional development.

Job Requirements:

1. Graduate from an approved nursing program.
2. Current RN licensure in the state of employment, with at least one year of clinical experience in an acute or long-term care setting; home health experience is preferred but not required.
3. Knowledge of fundamental medical-surgical nursing principles and practices.
4. Excellent clinical nursing skills.
5. Understanding of adult learning principles and medical record-keeping standards.
6. Effective oral and written communication skills.
7. Proficiency with computers and various applications.
8. Strong problem-solving, critical thinking, and reasoning skills for patient care planning and issue resolution.
9. CPR certification.
