Job Description

• Actively participate in the interdisciplinary care planning process.
• Provide professional nursing services according to the care plan established by the Patient Care Coordinator.
• Promote favorable outcomes through collaborative practice and appropriate resource use.
• Accurately document services to support interdisciplinary coordination, reimbursement, legal compliance, and quality improvement.
• Demonstrate positive interpersonal relationships.
• Take personal accountability for work and professional growth.

Job Requirements

• Successful completion of nursing education from an approved school.
• Current RN licensure in the state of employment, with at least one year of clinical experience in acute or long-term care (home health experience preferred but not required).
• Knowledge of medical-surgical nursing principles.
• Excellent clinical skills.
• Knowledge of adult learning principles and medical record standards.
• Effective communication skills, both oral and written.
• Proficiency in computer applications.
• Strong problem-solving, critical thinking, and reasoning skills.
• CPR certification.
