Job summary

Community Health Network (CHN) seeks an experienced Home Care Physical Therapist to serve the Haverhill, MA area and nearby towns.

This role focuses on Medicare home health visits with OASIS documentation, Monday–Friday, 40 hours per week, and every 6th weekend rotation. CHN offers competitive pay plus mileage reimbursement and comprehensive benefits (health, dental, vision, 401(k), life insurance).

Qualifications

• Minimum 1 year of recent Home Health Physical Therapy experience.
• Experience completing OASIS visits for Medicare-certified agencies.

Responsibilities

• Provide home health physical therapy services to patients in the Haverhill, MA area.
• Coordinate with care teams and document visits and outcomes as required.

Currently we are assisting one of our top home care agency partners with a need for an experienced Home Care Physical Therapist.




Coverage needs primarily in the Haverhill/Merrimack/Methuen/byfield/Georgetown, MA area and surrounding towns.

• Monday-Friday, 40 hours/week
• Every 6th weekend rotation
• Highly competitive pay rate plus IRS mileage reimbursement

Requires 1 year of recent Home Health Physical Therapy experience doing OASIS visits with a Medicare certified agency.
