Job summary

Community Health Network (CHN) is seeking an experienced Home Care Physical Therapist to serve patients in the Boston/Jamaica Plain/Dorchester area. This full-time role focuses on in-home visits with Medicare-certified oversight. Requires 1 year of recent home care PT experience doing visits in Home Health with a Medicare certified agency.

Monday–Friday, 40 hours weekly, 6th weekend rotation. Competitive pay with mileage reimbursement.

Qualifications

• 1 year of recent Home Health PT experience with a Medicare-certified agency.

Responsibilities

• Provide in-home PT evaluations and treatment plans for patients in Boston area.
• Collaborate with care team to coordinate services and ensure quality outcomes.

• Full-time, Monday-Friday, 40 hours/week
