Job summary

Community Health Network, Inc. is seeking an experienced Home Health Physical Therapist to serve patients in Perry County, PA and surrounding areas. This is a 40-hour-per-week role, Monday through Friday, with no weekends or holidays and a 13-week contract to start.

Requirements include Medicare-certified home health PT experience, proficiency with Homecare Homebase and OASIS, and the ability to coordinate care with a multidisciplinary team. Flexible options and competitive pay are offered.

Currently we are assisting one of our top home care agency partners with a need for an experienced Home Health Physical Therapist to see patients in Perry County, PA and surrounding area.

• Monday-Friday, 40 hours/week guaranteed
