Job summary

Community Health Network (CHN) is seeking an experienced Home Care Physical Therapist to support patients in the Chelsea/Winthrop/East Boston, MA area. The role is full-time, contract, with a 40-hour week and every 6th weekend rotation.

Requires at least 1 year of recent Home Health PT experience performing OASIS visits with a Medicare-certified agency. Benefits include health, dental, vision, 401(k) and company-paid life insurance.

Qualifications

• Minimum 1 year recent Home Health Physical Therapy experience performing OASIS visits for a Medicare-certified agency.

Responsibilities

• Provide home health physical therapy visits primarily in Chelsea/Winthrop/East Boston and surrounding towns.
• Support flexible scheduling and weekend rotation as stated in ad.
