Metro Physical & Aquatic Therapy is seeking a motivated per diem Physical Therapist to provide in-home care in Bergen County, NJ, including Teaneck, Bogota, and Paramus. The position offers 1099 and W-2 options with a flexible schedule and opportunities for professional growth.

Must be a Licensed Physical Therapist in New Jersey or eligible, with a Physical Therapist Program Graduate background. Certification in First Aid, AED, and CPR is preferred, and travel to patients’ homes is required to

Qualifications

• Must be licensed physical therapist in NJ or eligible.
• Graduated from Physical Therapist program.
• First Aid, AED, CPR certifications preferred.

Responsibilities

• Accept and treat patients within designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs.
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care.
• Educate family members, home health aides, and other caregivers on proper therapy techniques.
• Plan, and carry out therapy programs to maintain, improve or restore physical function and prevent dysfunction.
• Perform and document an initial exam, evaluating data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit.
• Evaluate effects of treatment and adjust treatments to maximize benefit.
• Must be willing to treat geriatric patients.

Education

Licensed Physical Therapist in New Jersey or eligible

• Contract
• Compensation: USD 72 - USD 95 - hourly

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New Jersey or eligible.
• PreferredCertifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory
