Job summary

Ignite Rehab, operating as part of Metro Physical & Aquatic Therapy, is seeking a per diem Occupational Therapist to provide in-home care in Ocean County, NJ, including Toms River. This role offers flexible scheduling and a mix of 1099 and W-2 options.

Responsibilities include 1:1 treatment, developing individualized plans, documenting visits, and traveling to patients' homes within the assigned geographic area.

Qualifications

• Occupational Therapist program graduate and licensed or eligible in NJ.
• Familiar with home health settings and patient education.
• Willingness to travel to patient homes within Ocean County.
• Must maintain required certifications (First Aid, AED, CPR).

Responsibilities

• Provide 1:1 occupational therapy in patients' homes.
• Evaluate patients and develop individualized treatment plans.
• Deliver care aligned with physician orders and policy.
• Educate family members and caregivers on therapy techniques.
• Document visits timely and accurately.
• Willing to treat geriatric patients.

Home Care Occupational Therapist- Per Diem (Ocean County)

• Part-time
• Compensation: USD 72 - USD 95 - hourly

Responsibilities:

• Accept and treat patients within your designated geographic area
• Provide 1:1 treatment
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
• Educate family members, home health aides, and other caregivers on proper therapy techniques
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Must be willing to treat geriatric patients.

• Occupational Therapist Program Graduate
• Licensed Occupational Therapist in the state of New Jersey or eligible.
• Mandatory Certifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients' homes within the assigned geographic territory
• Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.
