Job summary

Ignite Rehab (DBA Metro Physical & Aquatic Therapy) seeks a motivated per diem Physical Therapist to provide in-home care in Ocean County, NJ, including Toms River. The role offers flexible scheduling and both 1099 and W-2 options. You will work with a team of respected clinicians dedicated to patient-centered care.

All levels of experience are welcome; mentorship is provided for new graduates. Pay ranges from $72 to $95 per visit.

Qualifications

• Physical Therapist license is required or eligible in New Jersey.
• Minimum requirement: NJ PT license or eligibility to obtain one.
• Preferred certifications: First Aid, AED, CPR.

Responsibilities

• Accept and treat patients within designated geographic area.
• Provide 1:1 treatment.
• Evaluate patients and develop individualized treatment plans.
• Deliver care aligned with professional standards and plans of care.
• Educate family members and caregivers on therapy techniques.
• Plan and carry out occupational therapy programs to maintain or improve function.
• Perform and document initial exams and diagnoses prior to intervention.
• Provide timely documentation for each visit.
• Evaluate treatment effects and adjust as needed.
• Treat geriatric patients as required.

Responsibilities

• Accept and treat patients within your designated geographic area
• Provide 1:1 treatment
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
• Educate family members, home health aides, and other caregivers on proper therapy techniques
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Must be willing to treat geriatric patients.

Qualifications

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New Jersey or eligible.
• Preferred Certifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory
