Job summary

Ignite Rehab (DBA Metro Physical & Aquatic Therapy) is seeking a motivated Part-time Physical Therapist to provide in-home care in Bergen County, NJ. The role requires travel to patients' homes within the assigned geographic area and offers a flexible schedule with benefits.

You will perform 1:1 treatments, evaluate and create individualized plans based on physician orders, and document care per policy. Mentoring for documentation is provided.

Qualifications

• Must be a licensed Physical Therapist in New Jersey or eligible.
• Experience in home health is preferred but not required; mentorship provided for documentation/compliance.
• Ability to travel to patients' homes within the assigned geographic territory.
• Excellent communication with patients, caregivers, and management.

Responsibilities

• Accept and treat patients within geographic area.
• Provide 1:1 therapy sessions.
• Evaluate patients and develop treatment plans based on orders.
• Deliver care aligned with standards and policies.
• Educate family members and caregivers on proper techniques.
• Perform and document initial exam and determine diagnosis prior to intervention.
• Provide timely documentation for each visit.
• Evaluate treatment progress and adjust accordingly.
• Willing to treat geriatric patients.

We are looking for a motivated and passionate part time (with benefits!) Physical Therapist to provide in-home care. • Job Type: Part time w/ benefits, 20-visits per week
• Location: Bergen County (Including Fort Lee & Englewood)
• Schedule: Flexible!

• Accept and treat patients within your designated geographic area
• Provide 1:1 treatment
• Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
• Deliver care that aligns with professional standards, agency policies, and the prescribed plan of care
• Educate family members, home health aides, and other caregivers on proper therapy techniques
• Plan, and carry out occupational therapy programs to maintain, improve or restore physical function, relieve pain, and prevent occupational dysfunction in patients.
• Perform and document an initial exam, evaluating the data to identify problems and determine a diagnosis prior to intervention.
• Provide timely and accurate documentation for each visit
• Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit
• Must be willing to treat geriatric patients.

Qualifications

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New Jersey or eligible.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients' homes within the assigned geographic territory
• Strong communication skills between patients, support staff, and management, including timely responses via text, email, phone, etc.

The anticipated base salary range for this position is $60,000.00-$75,000.00. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Additionally, this position is eligible for incentive compensation. The Company's incentive compensation plan is subject to change. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
