Job summary

Community Health Network, Inc. is seeking an experienced Home Care Physical Therapist to join our team serving patients in North Andover and surrounding MA communities.

The role features a Monday–Friday schedule, 40 hours per week, with every 6th weekend rotation and no on-call requirements. You will deliver evidence-based PT in patients' homes, perform OASIS visits for Medicare-certified care, and collaborate with our multidisciplinary team.

Qualifications

• Must hold a current Physical Therapist license and be registered to practice in Massachusetts.
• Minimum 1 year of recent home care PT experience performing OASIS visits for a Medicare-certified agency.
• Strong documentation and communication skills required for coordinating care with families and providers.

Responsibilities

• Provide in-home physical therapy services across the North Andover, Topsfield, Hamilton, Danvers, Peabody, Lynnfield area.
• Complete OASIS visits and ensure Medicare compliance.
• Collaborate with case managers to optimize patient outcomes.
