The role requires at least 1 year of recent Home Care RN Case Management experience performing OASIS visits for a Medicare-certified agency, with a strong focus on patient and family outcomes and flexible scheduling options.

Qualifications

• Requires minimum 1 year of recent Home Care RN Case Management experience doing OASIS visits with a Medicare certified agency.

Responsabilités

• Manage and care for a caseload of patients and their families in the community from admission to discharge.
