Job summary

Community Health Network, Inc. in Massachusetts seeks an Occupational Therapist for per diem home health. You will assess patients, develop personalized care plans, and support ADLs, geriatrics, pediatrics, and post-acute recovery in Burlington/Woburn/Lexington areas.

Qualifications

Newer growing home care agency has a need for an Occupational Therapist home health experience on aPer Diembasis.




Position Details:

• Monday-Friday (open to some after hours and weekends if preferred)
• Coverage are would be in and around the Burlington/Woburn/Lexington MA areas
• Competitive Pay, Per Visit
• More regular hours may be available over time

Minimum 1-2 years of experience inHome Healthas an Occupational Therapist Registered with the Board of Registration.




This role involves developing personalized care plans, assisting patients with activities of daily living (ADLs), and supporting recovery and independence for individuals with diverse needs such as disabilities, geriatrics, pediatrics, and those recovering from acute or post-acute conditions. The therapist will collaborate with multidisciplinary teams to ensure comprehensive patient care and optimal outcomes.
