Job summary

Metro Physical & Aquatic Therapy is seeking a motivated PER DIEM Physical Therapist to provide in-home care on the Upper East Side of New York. Pay is $75-$90 per visit, with flexible scheduling and both 1099 and W-2 options to suit your work style.

Responsibilities include evaluating patients, developing PT plans, training families, and maintaining timely documentation, while traveling to patients' homes within your geographic area. NY-licensed or eligible candidates encouraged to apply.

Qualifications

• Physical Therapist Program Graduate.
• Licensed Physical Therapist in the state of New York or eligible.
• Mandatory Certifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - mentorship provided for documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning.
• Deliver care in compliance with agency policies and professional practice standards.

Additional Information
