Job summary

Metro Physical & Aquatic Therapy is seeking a motivated Home Care Physical Therapist to provide in-home services in Queens, NY on a part-time basis. This 1099/W-2 flexible role offers pay per visit and opportunities to grow within a multidisciplinary team.

Ideal candidates are licensed NY PTs or eligible, with graduation from an accredited PT program. Compliance with First Aid, AED, CPR is required; travel to patients' homes is expected and mentorship is available for new graduates.

Qualifications

• Must be a licensed Physical Therapist in NY or eligible.
• Graduate of an accredited Physical Therapist program.
• Mandatory certifications: First Aid, AED, CPR.
• Home health experience preferred but not required; mentorship provided.
• Ability to travel to patients' homes within the geographic territory.

Responsibilities

• Accept and treat patients within your designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans.
• Plan, prepare and implement customized therapy programs to restore function and promote independence.
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families, and interdisciplinary team members.
• Deliver care in compliance with agency policies and professional practice standards.

• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New York or eligible.
• Mandatory Certifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory

The anticipated pay range for this position is$70-90 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
