Job summary

Metro Physical & Aquatic Therapy is seeking a Part-time Home Care Physical Therapist to provide in-home services in Manhattan. The role offers 1099 and W-2 options, a flexible schedule, and pay of $75-$90 per visit, with training available for new graduates.

Qualifications

• - Physical Therapist Program Graduate.
• - Licensed Physical Therapist in New York or eligible.
• - Home health experience preferred but not required; mentor provided for documentation/compliance.
• - Ability to travel to patients' homes within geographic area.

Responsibilities

• Accept and treat patients within designated geographic area.
• Evaluate patients’ needs for physical therapy in home setting and develop treatment plans.
• Develop plan of care and goals with physician, patient, and family.
• Plan and implement customized therapy programs to restore function.
• Instruct family members, PTAs, HHAs, and caregivers in therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate with patients and families and the interdisciplinary team, especially discharge planning.
• Deliver care in compliance with policy and professional standards.

• Compensation: USD 75 - USD 90 - hourly

Responsibilities:

• Plan, prepare and implement customized therapy programs to restore function and promote independence
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
• Provide timely and accurate documentation for each visit
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New York or eligible.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory

The anticipated pay range for this position is$75-90 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
