Job summary

Metro Physical & Aquatic Therapy is seeking a motivated Physical Therapist to provide in-home care in Manhattan on a per diem/contract basis. The role offers 1099 and W-2 options and a flexible schedule to accommodate patients in the geographic area.

Licensed NY PT or eligible required; training provided for new graduates. Travel to patient homes within the assigned territory is expected, with opportunities for professional growth and a supportive team environment.

Qualifications

• Graduate of a Physical Therapist program and licensed or eligible in NY.
• Ability to travel to patients' homes within geographic area.
• First Aid, AED, and CPR certifications required.

Responsibilities

• Accept and treat patients within designated geographic area.
• Evaluate home-based PT needs and develop individualized plans of care.
• Plan, implement and monitor customized therapy programs to restore function.
• Instruct family members and caregivers on therapy techniques and home programs.
• Provide timely documentation for each visit and coordinate with physicians.
• Communicate effectively with patients and interdisciplinary team, especially on discharge planning.
• Deliver care in compliance with agency policies and professional standards.

Home Care Physical Therapist-1099 (Manhattan)

• Contract
• Compensation: USD 75 - USD 90 - hourly

Responsibilities:

• Accept and treat patients within your designated geographic area
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family
• Plan, prepare and implement customized therapy programs to restore function and promote independence
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
• Provide timely and accurate documentation for each visit
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New York or eligible.
• Mandatory Certifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory

The anticipated pay range for this position is$75-90 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
