Metro Physical & Aquatic Therapy is seeking a motivated and passionate Per Diem Physical Therapist to provide in-home care on the Upper East Side. All levels of experience are welcome and will train the right new graduate.

As a PT, you will evaluate patients, develop plans of care, and deliver therapy in the home setting. This role offers flexible scheduling, 1099 and W-2 options, competitive pay per visit, and a collaborative, patient-focused team.

Qualifications

• Physical Therapist Program graduate and licensed in NY or eligible.
• Home health experience preferred but not required; mentorship provided.
• Ability to travel to patients' homes within the geographic area.

Responsabilités

• Accept and treat patients within designated geographic area.
• Evaluate home-based PT needs and develop individualized plans of care.
• Plan and implement customized therapy programs to restore function.
• Instruct family members and aides on therapy techniques.
• Provide timely, accurate documentation for each visit.
• Communicate with patients, families and the care team about discharge planning.
• Deliver care in compliance with agency policies and standards.

Responsibilities

• Accept and treat patients within your designated geographic area
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family
• Plan, prepare and implement customized therapy programs to restore function and promote independence
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
• Provide timely and accurate documentation for each visit
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards

Qualifications

• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New York or eligible.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory

Additional Information

The anticipated pay range for this position is $75-90 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
