A dynamic healthcare organization is seeking a motivated Home Care Physical Therapist to provide in-home care in New York City. This position offers flexible scheduling and competitive compensation ranging from $65 to $80 per hour. Candidates must be licensed Physical Therapists in New York and possess mandatory certifications. The role involves evaluating patients, developing treatment plans, and instructing caregivers, all while delivering high-quality patient care in a supportive team environment.

Formación

• Licensed Physical Therapist in New York or eligible.
• Mandatory Certifications: First Aid, AED, & CPR.
• Home health experience is preferred but not required.

Home Care Physical Therapist-1099 (Manhattan)

• Compensation: USD 65 - USD 80 - hourly

Metro is looking for a motivated and passionate PER DIEM Physical Therapist to provide in-home care. Responsibilities:

• Accept and treat patients within your designated geographic area
• Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
• Develop a plan of care and treatment goals in consultation with the physician, patient, and family
• Plan, prepare and implement customized therapy programs to restore function and promote independence
• Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
• Provide timely and accurate documentation for each visit
• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards
• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New York or eligible.
• Mandatory Certifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients’ homes within the assigned geographic territory

The anticipated pay range for this position is $65-80 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
