Job summary

Metro Physical & Aquatic Therapy seeks a motivated Home Care Physical Therapist to provide in-home services throughout Queens. This PER DIEM role offers 1099 and W-2 options, with flexible scheduling and a focus on patient-centered care.

Licensed NY PT or eligibility is required, along with First Aid, AED and CPR certifications. Travel to patients' homes within the geographic area is expected and mentorship is available for new graduates.

Qualifications

• Physical Therapist Program Graduate.
• Licensed Physical Therapist in NY or eligible.
• Mandatory Certifications: First Aid, AED, CPR.
• Home health experience preferred but not required; mentorship provided.
• Ability to travel to patients' homes within geographic territory.

Responsibilities

• Accept and treat patients within designated geographic area.
• Evaluate patients’ needs for physical therapy in the home setting and develop treatment plans.
• Develop a plan of care and treatment goals with physician, patient and family.
• Plan, prepare and implement customized therapy programs to restore function.
• Instruct family members, PTAs, HHAs on therapy techniques.
• Provide timely and accurate documentation for each visit.
• Communicate effectively with patients, families and interdisciplinary team, especially discharge planning.
• Deliver care in compliance with policies and professional standards.

• Contract
• Compensation: USD 75 - USD 90 - hourly

• Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
• Deliver care in compliance with agency policies and professional practice standards
• Physical Therapist Program Graduate
• Licensed Physical Therapist in the state of New York or eligible.
• Mandatory Certifications: First Aid, AED, & CPR Certifications.
• Home health experience is preferred but not required - We will provide you with a mentor to help with documentation and compliance.
• Ability to travel to patients' homes within the assigned geographic territory

The anticipated pay range for this position is$75-90 per visit. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography.
