Job summary

BrightStar Care in Park City, Utah is hiring an RN or LPN to provide in-home care for a 19-year-old male with TBI. The caregiver will manage medication setup, carry out plans of care, and perform in-home assessments.

You will monitor changes, communicate with the Director of Nursing, collaborate with the family, provide personal care, and help maintain safety and comfort in the client’s home. We offer weekly pay, benefits, and opportunities to grow with a respected home care leader.

Qualifications

• Must have at least 1 year nursing experience with neurological and/or behavioral health.
• Must have at least 1 year nursing experience in home health.
• Current unrestricted RN or LPN license in Utah.
• Certified CPR and background check required.
• Negative TB skin test or chest x-ray.

Responsibilities

• Medication set up and oversight.
• Provide care based on personalized Plan of Care.
• Provide in-home assessments.
• Monitor and track changes and inform the Director of Nursing.
• Collaborate with family on plan of care, health changes and concerns.
• Total personal cares.

Client is a 19-year old male withTBI. He iswheelchair bound, limited speech, hemiplegic, neurological and/or behavioral health experience a must.

Available Shifts in Park City:

• Monday - Thursday 2:30 PM - 7 PM
• Friday 12 PM - 7 PM
• Saturday and Sunday 7am - 7pm

We are looking for an RN or LPN who is interested in the following:

• Medication set up and oversight
• Provide thoughtful care to clientsbased on personalized Plan of Care
• Provide in home assessments
• Monitor and track changes in the clientsconditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed
• Collaborate with client's family on plan of care, health changes, and concerns
• Total personal cares

RN and LPN Requirements

• Must have at least 1 year nursing experience with neurological and/or behavioral health
• Must have at least 1 year nursing experience in home
• Current unrestricted RN or LPN license in Utah
• Certified CPRCertification
• Negative TB skin test or chest x-ray
• Must be able to pass a drug test and a background check
• Licensed driver with valid automobile insurance
• Must be able to stand walk, squat, bend, kneel, reach, twist, push and pull.
• Must be able to lift 75 pounds
