Job summary

BrightStar Care of Salt Lake City East is seeking an RN to provide in-home care for a 19-year-old male with TBI in Park City. The role includes medication setup, care planning, and regular in-home assessments, with ongoing coordination with the family and nursing leadership.

You will work on multiple shifts, including weekday evenings and weekend coverage, with a focus on safety, dignity, and personalized care plans. Join a provider recognized for high standards and supported benefits.

Qualifications

• Must have at least 1 year nursing experience with neurological and/or behavioral health.
• Must have at least 1 year nursing experience in home health care.
• Current unrestricted RN license in Utah.
• Certified CPR.
• Negative TB skin test or chest x-ray.
• Must pass a drug test and a background check.
• Licensed driver with valid automobile insurance.
• Able to stand/walk, squat, bend, kneel, reach, twist, push and pull.
• Able to lift 75 pounds.

Responsibilities

• Medication setup and oversight.
• Provide thoughtful care based on personalized Plan of Care.
• Provide in home assessments.
• Monitor and track changes in client’s condition and notify Director of Nursing.
• Collaborate with client’s family on plan of care and health changes.
• Provide total personal care.

Client is a 19-year old male withTBI. He iswheelchair bound, limited speech, hemiplegic, neurological and/or behavioral health experience a must.

Available Shifts in Park City:

• Monday - Thursday 2:30pm - 7pm
• Friday, Saturday and Sunday 1pm - 7pm
• Saturday, Sunday & Monday 7pm - 7am

We are looking for an RN who is interested in the following:

• Medication set up and oversight
• Provide thoughtful care to clientsbased on personalized Plan of Care
• Provide in home assessments
• Monitor and track changes in the clientsconditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed
• Collaborate with client's family on plan of care, health changes, and concerns
• Total personal cares
