Qualifications

• Previous experience in home care, home health, healthcare coordination, Medicaid waiver services, case management, or related healthcare field.
• Working knowledge of Indiana Medicaid Waiver Program, waiver services, referral pathways, eligibility processes, and service coordination.
• Experience communicating with Medicaid waiver case managers, care coordination agencies, healthcare providers, or community-based service organizations.
• Excellent interpersonal, written, and verbal communication skills.
• Strong organizational skills, attention to detail, and follow-through.
• Ability to manage multiple referrals and priorities in a fast-paced environment.
• Proficiency in Microsoft Office, email, and electronic documentation or client management systems.
• Valid driver's license and reliable transportation if community outreach is required.

Responsibilities

• Develop and maintain referral relationships with hospitals, physician offices, social workers, discharge planners, case managers, and other sources.
• Cultivate relationships with Indiana Medicaid waiver case managers, waiver service providers, care coordination agencies, and community organizations.
• Identify new referral opportunities to increase awareness of home care services.
• Conduct outreach visits, presentations, and follow-ups with referral partners.
• Represent the organization at community events and healthcare-related functions.
• Assist with intake documentation and coordinate with internal departments.
• Monitor referral progress and minimize delays in starting services.
