Enhabit Home Health & Hospice is seeking a dynamic relationship manager to foster referral partnerships with physicians, hospitals and health systems. You will educate stakeholders on services and program requirements, supporting post-acute care transitions to improve patient outcomes and satisfaction.

This role emphasizes strong communication, organizational skills, and a deep understanding of Medicare home health operations, with in-office attendance being essential to achieve objectives.

Qualificações

• Bachelor’s degree or higher in healthcare administration, marketing or related field, or equivalent work experience.
• Strong understanding of healthcare regulations and Medicare home health operations.
• Two+ years in healthcare business development, community relations, marketing, or sales.

Responsabilidades

• Act as primary relationship manager between Enhabit and referral sources such as physicians and hospitals.
• Educate referral sources and patients about Enhabit services to drive referral growth.
• Navigate post-acute care transitions to improve patient outcomes and satisfaction.
