Job summary

Ashealthnet is seeking an experienced Registered Nurse (RN) Hospital Liaison in Indianapolis, IN. The role focuses on coordinating patient transitions from hospital to home care and supporting discharge planning with the health system.

The RN will participate in multidisciplinary rounds, determine home care eligibility, review medical documentation, and advocate for patients while building strong facility relationships.

Qualifications

• Must be a Registered Nurse with an active Indiana license.
• Minimum of two years of nursing experience.
• Home health experience preferred.
• Valid driver's license and auto insurance in the applicant's name.

Responsibilities

• Coordinate home health care services as ordered by the attending physician.
• Attend discharge and multidisciplinary rounds to support patient transitions.
• Determine home care eligibility and review patient insurances and medical documentation.
• Act as a patient advocate and maintain relationships with facilities and families.
