Responsibilities

• Follow up on referrals from marketing representatives and referral sources.
• Manage the intake process for patients seeking home care services.
• Coordinate and follow up on patient approvals with insurance companies and other vendors.
• Communicate regularly with patients, families, referral sources, physicians, and outside vendors.
• Maintain accurate and up-to-date patient records and intake documentation.
• Ensure all required documentation is collected and submitted timely.
• Monitor referral status and provide updates to patients and referral sources.
• Assist with staffing and case coordination when needed.
• Work closely with intake, coordination, and management teams to ensure timely service initiation.
• Meet departmental goals and performance expectations.
