Job summary

Personal Home Care of NC is seeking a Home Care Services Coordinator and Liaison to act as the primary relationship manager between the agency and referral sources in Gastonia.

The role involves overseeing daily operations, in-home aide recruiting and scheduling, coordinating with caregivers, ensuring client files and regulatory compliance, generating referrals, and promoting services to support census growth.

Qualifications

• Bachelor's degree or equivalent experience in a healthcare/administrative field.
• Experience in healthcare/home care administration or business development preferred.
• Strong communication and relationship-building skills required.

Responsibilities

• Develop and maintain relationships with healthcare providers, social workers, case managers, and staff.
• Coordinate daily operations of the home care agency.
• Educate referral partners, clients and families about services and programs.
• Generate referrals and support census growth goals.
• Coordinate with caregivers to ensure scheduling and information needs are met.
