TheKey is seeking a Home Care Liaison to be the face of our services in the community, driving referral partnerships and growing a thriving book of business while ensuring high-quality care for clients. This role emphasizes empathy, effective communication, and strong relationship-building with families and partners.

As part of our team, you will collaborate with internal teams to tailor care solutions, meet ambitious revenue targets, and champion our mission to transform aging at home.

Qualifications

• One plus years of sales experience preferably in health care or related field.
• Excellent customer service and sales skills.
• Computer proficiency and ability to document sales activity timely and accurately.

Responsibilities

• Build and maintain strong relationships with community referral sources and represent the organization at events.
• Respond to prospective client inquiries, conduct assessments, and convert prospects into clients.
• Coordinate with internal teams to ensure care plans meet client needs and timely sharing of information.
• Provide active relationship management for existing cases through home visits and meetings.
• Deliver after-hours support for referral partners, prospects, and active clients.
• Monitor regulatory compliance and market opportunities.
