Qualifications

• PHC, MC experience preferred

Responsibilities

• Completes visits as scheduled and reports daily to scheduler.
• Recruits, hires, trains, and supervises attendants.
• Completes appropriate forms and attaches to the daily visit report; submits within 48 hours.
• Maintains confidentiality of clients, attendance, and staff information.
• Participates in staff meetings, committees, and community relations activities.
