Job summary

Girling Personal Care, a member of the Addus Family of Companies, is seeking a Field Supervisor to conduct on-site client evaluations and develop service plans with authorization, performing other field visits as assigned.

You will complete visits as scheduled and report daily to the scheduler, recruit, hire, train, and supervise attendants, and attach forms to daily visit reports for 48-hour submission to the office.

Qualifications

• Two years of college study required; HS diploma may substitute one year of supervisory experience.
• PHC, MC experience preferred.

Responsibilities

• Complete visits as scheduled and report daily to scheduler.
• Recruit, hire, train, and supervise attendants.
• Attach forms to the daily visit report and file with the office within 48 hours.
• Maintain supervisor files and documentation of pertinent information.
• Participate in staff meetings and in-services and community relations activities.
• Maintain confidentiality of clients, attendance and staff information.
