Job summary

Harbor Health Services, Inc. seeks a Home Care Coordinator to support the Nurse Manager and Home Care team in Mattapan, MA. You will handle referrals, scheduling, and coordination with Home Care Nurses and Aides, building trust with clients and families to ensure quality care.

This is a full-time, 40-hour-per-week position with a comprehensive benefits package including health, dental, vision, life and disability insurance, a 403b plan, generous PTO and holidays.

Qualifications

• High School Diploma or GED required.
• Two (2)+ years of work experience in home care, professional or business environment.
• Excellent communication and customer service skills.
• Office 365 applications with focus on Word, Excel, Outlook and Teams.
• Associates Degree in Healthcare or Business preferred.
• Understanding of Home Care services preferred.
• Certified Nurse Assistant (CNA) preferred.

Responsibilities

• Receives referrals for clients in need of home health services.
• Builds trust and fosters smooth relationships with clients and families.
• Communicates with Home Care Nurse and Clinical Nurse Manager as needed.
• Interacts with clients and families with high standards of care and professionalism.
• Adjusts home aide schedules for sick calls, vacations and absences.
• Matches participant needs with home care aide strengths.
• Distributes and files home care assessments.
• Processes monthly home aide mileage reimbursement forms.
• Tracks Home Health Aide attendance for payroll submission.
• Files and copies documents as needed.
