Qualifications

• High School Diploma or GED required.
• CNA preferred.
• 2+ years in home care or related setting.
• Excellent communication and customer service skills.
• Proficient in Word, Excel, Outlook and Teams.
• Associates degree in healthcare or business preferred.
• Understanding of home care services preferred.

Responsibilities

• Receives referrals and coordinates home health services.
• Schedules and coordinates home care aides.
• Communicates with the Nurse Manager and care team.
• Builds trust with clients and families.
• Adjusts schedules for sick calls and vacations.
• Matches client needs with home care aide strengths.
• Distributes and files home care assessments.
• Processes mileage reimbursement forms.
• Tracks Home Health Aide attendance for payroll.
• Maintains professional, compassionate client relationships.
