Job summary

AltaMed is seeking a Home Care Coordinator to support and implement home care services for participants. You will coordinate with caregivers, vendors, and equipment, while acting as a liaison between the IDT and providers to ensure safe, high-quality care.

Qualifications include CA CNA certification, at least 1 year of healthcare experience, a valid driver’s license, and bilingual abilities (English plus Spanish/Mandarin/Cantonese). BLS certification is required.

Qualifications

• CNA certification issued by the State of California.
• Minimum 1 year of CNA/healthcare experience, preferably with frail/elderly.
• Driver's license required.
• Bilingual ability: English plus Spanish, Mandarin or Cantonese preferred by location.
• Minimum BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Support and implement home care services for participants.
• Coordinate with home caregiver vendors and family caregivers; arrange equipment and nursing home care.
• Act as liaison between the IDT and home care providers; demonstrate leadership and communication.
