Job summary
AltaMed Health Services Corporation in Lynwood, CA is seeking a Home Care Coordinator to support home care services, assess safe home environments, and coordinate with home caregiver vendors and family caregivers. Qualifications
- Graduation from an accredited CNA program with a current California CNA certification.
- Minimum of 1 year of experience as a CNA or healthcare worker, preferably with frail or elderly population.
- A valid Driver's License is required.
- Bilingual abilities: English/Spanish/Mandarin/Cantonese depending on location.
Responsibilities
- Provide support and implementation of home care services for participants.
- Assess safe home environments through home visit assessments and coordinate with home caregiver vendors and family caregivers.
- Coordinate durable medical equipment and nursing home care; attend scheduled IDT and coordination meetings.
- Act as liaison between the IDT and home care providers with strong leadership and communication.
