AltaMed Health Services Corp. is seeking a Home Care Coordinator to support and implement home care services for participants, coordinating with caregivers and vendors. The role includes evaluating safety at home and liaising with IDT teams.

The position requires CA CNA certification and 1+ year healthcare experience, with bilingual English/Spanish/Mandarin/Cantonese preferred. A driver’s license is required, and compensation is hourly with potential bonuses.

Qualifications

• CNA certification issued by the State of California is required.
• Minimum of 1 year of experience as a CNA or healthcare worker.
• Experience with frail or elderly population preferred.
• A valid driver’s license is required.

Responsibilities

• Support and implement home care services for participants.
• Evaluate safety of home environment through visit assessments.
• Coordinate with home caregiver vendors and family caregivers.
• Coordinate durable medical equipment and nursing home care.
