Job summary

AltaMed is seeking a Home Care Coordinator to support and implement home care services. You will evaluate living environments, coordinate with caregivers and vendors, and handle durable medical equipment coordination while attending IDT meetings.

Responsibilities include acting as a liaison between the IDT and home care providers, and assessing participants’ medical and social needs to tailor home care plans. Driver’s license and California CNA certification are required.

Qualifications

• CNA certification issued by the State of California is required.
• Minimum of 1 year experience as CNA or healthcare worker, elderly population preferred.
• Driver’s license required.
• Bilingual English/Spanish/Mandarin/Cantonese, depending on location.

Responsibilities

• Support and implement home care services for participants.
• Evaluate safe home environment via home visit assessments and coordinate with vendors.
• Coordinate durable medical equipment and nursing home care; attend IDT and meetings.
• Act as liaison between IDT and home care providers and identify participants’ needs.
