AltaMed Health Services in El Monte, CA is seeking a Home Care Coordinator to support home care services, assess safe home environments, coordinate with caregiver vendors and families, and participate in IDT meetings.

The role requires a California CNA certification, at least 1 year of healthcare experience, a driver's license, and bilingual abilities (English/Spanish/Mandarin/Cantonese). Compensation is $25.00 hourly, with potential bonuses.

Qualifications

• Graduation from an accredited CNA program with a current CNA certification in California.
• Minimum of 1 year of experience as a CNA or healthcare worker; elder care preferred.
• A valid driver's license is required.
• Bilingual English/Spanish/Mandarin/Cantonese preferred depending on location.
• Valid BLS certification or higher per AHA/Red Cross guidelines.

Responsabilités

• Support and implement home care services for participants.
• Evaluate safe home environments through home visit assessments and coordinate with caregivers.
• Coordinate with durable medical equipment and nursing home care; attend IDT/coordination meetings.
• Act as liaison between IDT and home care providers with strong leadership and communication.
