Job summary

AltaMed Health Services Corporation invites applications for a Home Care Coordinator. The role involves supporting and coordinating home care services, evaluating safety in the home, and liaising with caregivers, vendors, and IDT teams to ensure comprehensive patient care.

The position requires a California CNA certification, at least 1 year of healthcare experience, a valid driver’s license, and BLS certification. Bilingual ability in English and additional languages is preferred.

Qualifications

• Graduation from an accredited CNA program with a current California CNA certification.
• Minimum 1 year of CNA or healthcare experience, preferably with frail or elderly populations.
• Driver's License is required.
• Bilingual ability: English plus Spanish/Mandarin/Cantonese depending on location preferred.
• Valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Assist in evaluating safe home environments through home visit assessments.
• Coordinate with home caregiver vendors and family caregivers, and with DME and nursing home care.
• Attend scheduled IDT and coordination meetings and act as liaison between IDT and home care providers.
• Identify medical and social needs to evaluate and plan home care for participants.
