Job summary

AltaMed Health Services is seeking a Home Care Coordinator to support and implement home care services for participants in our program. The role involves evaluating home environments, coordinating with caregivers and vendors, and ensuring appropriate equipment and services are in place.

You will attend IDT meetings and act as the liaison between teams and providers. Ideal candidates hold California CNA certification, have at least 1 year of healthcare experience with elders, possess a valid

Qualifications

• Graduation from an accredited certified nursing assistant program with a current CNA certification issued by the State of California.
• Minimum of 1 year of experience practicing as a CNA or healthcare worker, and experience working with a frail or elderly population preferred.
• A driver's License is required.
• Bilingual English/Spanish/Mandarin/Cantonese, depending on location preferred.
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Responsibilities

• Support and implement home care services for participants.
• Evaluate safety of home environment through home visit assessments and coordinate with home caregiver vendors and family caregivers.
• Coordinate durable medical equipment and nursing home care; attend IDT and coordination meetings.
• Serve as liaison between the IDT and home care providers; demonstrate leadership and strong communication skills.
• Identify and understand participants’ medical and social needs to evaluate home care requirements.

Job Overview

The Home Care Coordinator is responsible for the support and implementation of home care services for participants. Provide support with the evaluation of a safe home environment through home visit assessments and coordination with home caregiver vendors and family caregivers, coordination of durable medical equipment, and nursing home care. Attends scheduled IDT and Coordination meetings. Responsible for acting as the liaison between the IDT and the home care providers, she or he should possess good leadership and communication skills. In addition, the Home Care Coordinator should be able to identify and understand participants’ medical and social needs to evaluate the home care needs of participants.

Minimum Requirements

Compensation

$25.00 - $25.00 hourly
