Job summary

AltaMed Health Services Corp. in El Monte, CA is seeking a Licensed Vocational Nurse, Home Care Coordinator to develop and implement homecare services for program participants and to coordinate all contracted home services, equipment, and nursing home care.

The ideal candidate holds a CA LVN license, has at least one year of LVN experience, a valid CA driver’s license, current BLS certification, and will attend IDT and coordination meetings.

Qualifications

Graduation from an accredited LVN program with a California LVN license.
Minimum of 1 year of LVN experience.
Experience with the physical, mental, and social needs of patients is desirable.
At least 1 year of working with a frail or elderly population.
Valid California driver’s license with a good driving record.
A valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

Develop and implement homecare services for program participants.
Coordinate all contracted home services, durable medical equipment, and nursing home care.
Attend scheduled IDT and coordination meetings.
