Job summary

AltaMed Health Services Corp. is seeking a Home Care Coordinator in California to support, assess, and coordinate home care services for participants.

You will liaise with home caregiver vendors and family caregivers, coordinate durable medical equipment, and join IDT meetings to ensure seamless care delivery. The role requires a California CNA certification, at least 1 year of healthcare experience, a valid driver’s license, and BLS or higher.

Qualifications

• Graduation from an accredited CNA program with current CA CNA certification.
• Minimum of 1 year experience as a CNA or healthcare worker; elder/frail population experience preferred.
• Driver's license required; BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Support and implement home care services for participants.
• Conduct home visit assessments to evaluate safe home environments.
• Coordinate durable medical equipment and nursing home care.
• Attend scheduled IDT and Coordination meetings; liaise between IDT and home care providers.
• Identify participants' medical and social needs to tailor home care plans.

Job Overview

The Home Care Coordinator is responsible for the support and implementation of home care services for participants. Provide support with the evaluation of a safe home environment through home visit assessments and coordination with home caregiver vendors and family caregivers, coordination of durable medical equipment, and nursing home care. Attends scheduled IDT and Coordination meetings. Responsible for acting as the liaison between the IDT and the home care providers, she or he should possess good leadership and communication skills. In addition, the Home Care Coordinator should be able to identify and understand participants’ medical and social needs to evaluate the home care needs of participants.

Minimum Requirements

Graduation from an accredited certified nursing assistant program with a current CNA certification issued by the State of California is required. Minimum of 1 year of experience practicing as a CNA or healthcare worker, and experience working with a frail or elderly population preferred. A driver's License is required. Bilingual English/Spanish/Mandarin/Cantonese, depending on location preferred. A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Compensation

$25.00 - $25.00 hourly
