AltaMed Health Services Corp. seeks a Licensed Vocational Nurse, Home Care Coordinator in El Monte, CA. The role will develop and implement home care services for program participants, coordinating contracted home services, DME, and nursing home care, and attending IDT/coordination meetings.

The position requires an LVN license in California, at least one year of LVN experience, a valid CA driver’s license, and a minimum of BLS certification. Local travel may be involved.

Qualifications

• Current California LVN license.
• At least 1 year of LVN nursing experience.
• CA driver’s license with clean driving record.
