Always Compassionate Home Care in Bala Cynwyd, PA is seeking a Case Coordinator to join our coordination team. The coordinator will work directly with caregivers to ensure clients receive compassionate, high-quality in-home care and will focus on scheduling and ongoing communication with clients and caregivers.

The ideal candidate has 3–5 years of home care experience, strong organizational and communication skills, and proficiency with technology.

• Strong customer service background with staff coordination.

Responsibilities

• Staff personnel according to home care patient schedules.
• Document and report changes to Case Manager for client status, service needs, or incidents.
• Maintain efficient time use and a fast-paced workflow.
• Communicate schedule changes and service updates to clients and families.
• On-call after hours during weekdays and weekends.
