Job summary

Open Arms Home Care, Inc. in the Philadelphia area is seeking a Care Coordinator to join our home care team. The ideal candidate will be experienced, organized, and dependable, coordinating services between clients and caregivers.

We offer a competitive salary of $45,000–$55,000 (commensurate with experience), vacation, and a safe working environment. The role involves supporting care planning for clients with complex conditions, referring clients to community resources, and documenting progress

Qualifications

• Minimum 2 years' experience in Home Care or related field.
• Knowledge of Home Care Regulations and timelines.
• Excellent time management skills and ability to work as part of a team.
• Familiar with office equipment such as computers, scanners, copiers, printers and fax machines.

Responsibilities

• Support care planning for clients with complex and chronic conditions, including performing and arranging for waiver services.
• Refer clients to community resources, agencies, and organizations to enhance and supplement services for the client.
• Document progress notes related to client contacts, advocacy, and care in accordance with Open Arms Home Care policies.
• Work with appropriate resources on evaluations for clients in need of discharge planning, transitions of care, or additional services.
• Facilitate authorizations and orders to coordinate services for clients.
• Develop and maintain relationships and knowledge of applicable community services.
• Facilitate patient access to recommended services.
