Job summary

AccentCare, Inc. is seeking a Field Supervisor for Personal Care Services in Illinois. This on-site role involves supervising Home Care Aides, ensuring care plans are followed, and conducting site visits across Will and Cook Counties.

Must have a valid driver’s license and reliable transportation. Responsibilities include coaching staff, evaluating service quality, and coordinating with Care Coordinators. Excellent communication and problem-solving skills are essential.

Qualifications

• High School Diploma or GED required.
• 2 years of Home care, caregiving or CNA experience.
• Strong communication, organizational, and problem-solving skills.
• Must have valid driver’s license, reliable transportation, and auto insurance.
• Bilingual skills a plus, but not required.

Responsibilities

• Provides supervision and leadership to Home Care Aides
• Evaluate the quality and effectiveness of services for each client served by HCA
• Conduct unannounced quarterly home visits with clients that have Paid Family Caregivers to ensure the plan of care is being followed and HCA's are present during the hours scheduled.
• Coach, counsel, mentor and evaluate the performance of HCA's
• Conduct home visits as necessary, to provide hands on training and assistance
• Initiate and/or participate in client-staffing discussion with Care Coordinator
• Investigate allegations of fraud and theft reported by HCA
• Documentation and follow up on incident reports
